Please assist with the following:
My late wife, who passed away in 2021, had a life insurance policy on her name. The life insurer, supplied a beneficiary nomination form, which stated, that she allocated 50% of the fund to her estate.
I dispute this nomination form, as there is no signature on the actual nomination form. However they have attached a signed form which states protection of personal information to the nomination form and claims it is the beneficiary nomination document.
This second form only has a signature, it is not dated and there is no contract number displayed on it.
Please advise on legal requirements with regards to nomination forms and what action I can take
